Membership Application Form

PLEASE NOTE Karate is a physically demanding, contact sport. As there is a risk of injury from participating in karate training and competitions, APPLICANTS PARTICIPATE IN ACTIVITIES ORGANISED BY THE CENTRE AT THEIR OWN RISK.

Terms and Conditions


The Applicant ACKNOWLEGES and AGREES that:


1. By signing this application form the Applicant agrees to be a member of the Centre and of the International Kyokushinkai Karate Australia (“the I.K..KA”) and their membership of the Centre and of the I.K.K.A and participation in the Activities is subject to these Terms and Conditions and any rules or guidelines as implemented or amended by the Centre or I.K.K.A from time to time.


2. For the purposes of these Terms and Conditions:


“Activity” or “Activities” means any karate practice, karate grading, karate tournament, participation or assistance in any karate demonstration, participation in any karate training camp, any self defence or martial arts training whether defined as Kyokushin Karate or otherwise, weight lifting and any other activity classified as sport or exercise that is organised by the Centre or the I.K.K.A.


“Assumed Risk” means any accident, loss, damage, injury, or death sustained by any person or to any property howsoever or whomsoever caused by or in connection with the Applicant’s participation in the Activities.


3. If the Applicant breaches any of these Terms and Conditions, the Centre and the I.K.K.A may at their absolute discretion cancel the Applicant’s membership.


4. The Applicant is aware of no physical or medical reason, and warrants that they have not at any time suffered any injury or physical or medical condition that would prevent them from undertaking strenuous physical exercise and forceful physical contact during the Activities.


5. The Applicant’s participation in the Activities is at their own risk and they release the Centre, the I.K.K.A and the Centre and the I.K.K.A’S employees and representatives as fully as permitted by law from all claims and demands for personal injury, death or property damage arising from the Applicant’s participation in the Activities.

6. The Applicant will indemnify the Centre and the I.K.K..A and keep the Centre and the I.K..KA indemnified against any claim, loss, damage, liability, or cost suffered or incurred by the Centre or the I.K.KA arising from or in respect of:


(a) the Applicant’s participation in the Activities:

(b) any breach by the Applicant of these Terms and Conditions

(c) any breach by the Applicant of any rules as implemented or varied by the Centre or the I.K.K.A from time to time; and

(d) any Assumed Risk.


7. The Applicant has never been convicted of any serious criminal offence and that if they are convicted of a serious criminal offence at any time during the membership, the Centre at its absolute discretion terminate the Applicant’s membership of the Centre and the I.K.K.A.


8. The Applicant must always ensure that that their fingernails and toenails are trimmed to a length that will prevent injury such as scratching and cutting to other members or opponents.


9. The Applicant must immediately cease participation in the Activities if they sustain any bleeding wound. The Applicant may not recommence any Activity until such time as in the opinion of a representative of the centre or the I.K.K.A that the wound has been properly treated, including treatment by a qualified medical practitioner if required, has been properly covered and poses no risk of being re-opened by the Applicant’s participation in the Activity.


10. The Applicant must undertake Hepatitis A, B C and HIV test before competing in their first full contact tournament. Applicants are required under state legislation to undertake these tests at least every two years if they are competing in contact tournaments.


11. The Applicant must not discriminate against any other member, employee, official or spectator on the grounds of gender, age, ethnicity, religion, and sexual preference. The Centre may in their absolute discretion cancel the membership of any Applicant who is found in breach of this clause.


12. The Centre shall not disclose any personal information obtained for the Applicant to any other person or organisation except as required by law of if such disclosure is required as part of Applicant’s participation in the activities.


13. The Applicant has read the Terms and Condition and is aware of the risks associated with participation in the Activities. The Applicant agrees that their membership of the Centre and the I.K.K.A and participation in the Activities is ENTIRELY AT THEIR OWN RISK and that they will be bound by these Terms and Conditions.
